Select a year and enter a license number

Dog licenses are formatted as #####, ex: 00099.
Kennel licenses are formatted as K####, ex. K0099.
Permanent licenses are formatted as P####, ex. P0099.
For permanent tag numbers, the Year entry is ignored.
